INCIDENT OR INJURY REPORT
To report an injury or incident on FVTC property immediately, call Security Services at (920) 735-5691. To access the incident report form us the link to the right, it takes you to the Human Resources page, scroll down the quick links on the right side of the page to find the incident report form.
FVTC Health Service provides to students currently enrolled:

Emergency first aid care.

Treatment for minor illness and injury so the student can return to regular activity, reach home safely, or be referred to a health care provider.

Tuberculin skin testing

Immunizations for measles, mumps, rubella (MMR), tetanus, and hepatitis B.
FVTC Health Service offers services to current students who:

Are concerned about a health problem

Need assistance with a referral to a healthcare provider or other community agency
IMMUNIZATION RECORDS
If you received an immunization at FVTC your record is available at the Wisconsin Immunization Registry (WIR). WIR is a great resource when looking for your shot record no matter where you received your shots within the state of Wisconsin.

LifeLINE Resource Center—Room E121
The College provides individual counseling through the LifeLINE Assistance Program related to longer term alcohol and drug abuse, and mental health issues. The ATODA Prevention Facilitator and therapist is , Wolfgang Wallschlaeger, MSE, CSAE, ICS. His office is located in the Counseling Center, Room E121 and may be contacted at (920) 735-5696.
Fitness Center—Room E170
The Fitness Center on the Appleton campus, features exercise equipment for use by students
free of charge
. Included in the center are aerobic and strength training equipment, as well as shower and locker facilities. Contact Student Life for more information (920) 996-2884.
