Start, Stop, Continue Form

The Start, Stop, Continue Form is designed for use by the students anytime during the semester. Instructors typically keep a stack of the forms available during every class session so that when students think of something they would like the instructor to start doing, stop doing, or continue doing they can give that feedback in time to possibly effect change in the class. The forms are designed to be anonymous and are not intended to replace the end-of-semester evaluations. Of course, the final decision regarding actions on the suggestions (if any) rests with the instructor.
